INDIANA SCHOOL FOR THE BLIND AND VISUALLY IMPAIRED
Policy S-09
The purpose of this policy is to ensure 1) proper response to and treatment of student injuries, 2) collection of data on accidents and mishaps to be used in pattern analysis to prevent or reduce risk, 3) notification of parents and appropriate staff members; 4) proper and timely payment of medical obligations, and 5) parents and guardians complete the Emergency Medical Authorization State Form 44083 on an annual basis before their child is enrolled at ISBVI. It is the policy of the Indiana School for the Blind and Visually Impaired that students may not be enrolled at ISBVI until the parents sign the Emergency Medical Authorization Form for student safety and liability purposes.
The ISBVI Board directs the ISBVI Superintendent to ensure ISBVI staff, contractors, volunteers, and parents comply with this policy by:
1) Developing ISBVI Administrative Directives and Procedures to implement the ISBVI Board Policy on Student Accidents and Injuries serving as notice to staff, volunteers, and parents clearly delineating their responsibilities, expectations, and procedures;
2) Providing ISBVI staff, contractors, volunteers, and parents written notice and documenting their receipt of these documents by obtaining their signatures;
3) Affording initial and periodic on-going training to ISBVI staff, contractors, and volunteers on the ISBVI Board Policy on Student Accidents and Injuries and accompanying ISBVI Administrative Directives and Procedures.
